WHAT WE DO
Through Early Clinical Intervention
Changes in condition can escalate quickly in older adults, often leading to unnecessary emergency room visits and hospital admissions. Our Hospital Avoidance Program focuses on early assessment, prompt provider response, and coordinated care to help facilities address concerns before they become emergencies.
We work alongside facility staff, primary care providers, home health agencies, and hospice teams to support safe, appropriate care within the resident’s community whenever possible.

Advanced Wound Care
Pressure injuries, skin tears, venous ulcers, diabetic foot ulcers, surgical wounds, moisture-associated skin damage.


Primary Care
Routine medical visits and urgent evaluations available. Services include chronic disease management, medication optimization, and more.

Clinical IV Therapy
Provider-directed IV hydration and infusion support to treat dehydration and other conditions within the facility, helping reduce unnecessary emergency room transfers.


Hospital Avoidance
Early provider evaluation for sudden changes in condition, infection concerns, dehydration, or worsening wounds. Our approach focuses on identifying problems early and stabilizing residents safely within the facility when appropriate.


Staff Education
Training and guidance for assisted living staff on wound prevention, skin assessment, dressing techniques, and recognizing when a resident needs provider evaluation.

Find Your Way to Us
We partner with assisted living teams to provide timely clinical support for residents who need wound care, medical evaluation, or treatment within the facility. Our process is designed to be simple for staff while ensuring residents receive prompt, professional care.

Discover
When a resident develops a wound, skin breakdown, sudden change in condition, or requires medical evaluation, your team can initiate a referral. Early evaluation helps prevent complications and allows residents to receive care without unnecessary hospital visits.

Connect
Submit a referral through our secure online form, fax, or phone. Our clinical team reviews the referral and coordinates directly with your facility to schedule the visit and gather any relevant clinical information.

Experience
Our provider evaluates the resident on-site, develops a clear treatment plan, and provides written care instructions for staff. Follow-up visits and care coordination are arranged as needed to support healing, resident comfort, and continuity of care.
