Skin Assessment & Pressure Injury Prevention
Early identification is one of the most important steps in preventing wound complications. Our wound care team provides staff education on routine skin checks, pressure injury risk factors, common areas of breakdown, and early warning signs that may require provider evaluation.
Training may include:

Dressing Application & Wound Monitoring
Consistent wound care helps support healing and reduces confusion among staff. Our team provides education on following written wound care protocols, understanding dressing frequency, monitoring drainage, and recognizing signs that a treatment plan may need to be adjusted.

Escalation, Documentation & Care Coordination
Assisted living staff need clear guidance on when to notify a provider, when to monitor, and when emergency care may be needed. Our wound care team helps staff understand escalation pathways, documentation expectations, and communication with families, PCPs, Home Health, and Hospice when appropriate.
